Applicants are requested to bring one set of photocopies of relevant documents mentioned below along with original certificates so that documents are properly verified:-
1. Certificate of Matric and 10+2 or equivalent issued by any recognized Board of School Education.
2.BAMS recognized degree in Ayurveda of not less than 5½ years duration from a recognized University/ Board established by law or institution recognized by the Central /State Govt. /NCISM along-with year wise marks sheet.
3.Certificate of Compulsory one year rotatory internship required by the NCISM. Registration certificate as Ayurvedic Practitioner from .P. Board Ayurvedic and Unani System of Medicines.
4.Latest certificate of Bonafide resident of H.P. Latest certificate belonging to ST where applicable.
5.Disability Certificate mentioning Nature of Disability/ Percentage. Registration certificate of Employment Exchange, if registered.
6.Two attested copy of recent passport size photograph (Coloured).
7. Latest Character Certificate issued by the Executive Magistrate Not more than six months old) Aadhaar Card (Photo copy) Certificate of higher qualification, if any.
Note:- No candidature will be considered without disability certificate i.e. OH, HI, VI(ST Category) & Multiple Disability. No TA/ DA will be provided by the Department.
