Notice Regarding filling of posts of Pharmacy Officer(Allopathy)through Batch wise :- HP Health Department

It is intimated that your names have been sponsored by the Director. Youth Services & Sports, HP, Shimla-02 to the post of Pharmacist now as Pharmacy Officer (Allopathy) Batch wise on contract basis. The Director. Youth Services & Sports, HP Shimla-02 also intimated that it will be the sole responsibility of this Directorate to verify age. educational qualification and any other condition applicable to the post. You are, therefore, directed to appear in person Directorate of Health and Family Welfare Department, Kusumpti Shimla -171009 on 31.01.2024 sharp at 11:00 AM alongwith relevant original documents & one set of photocopy of following documents for document verification:-

Detail of required documents is as under:-

1. Certificate of Employment Exchange Registration / Youth Services and Sports Cell. 2.Certificate of matriculation for age verification and if diploma done in Pharmacy Prior to 12.09.2001.
3. Certificate of 10+2 in Science , Degree or Diploma in Pharmacy from a recognized Board/ University alongwith Detail of Marks Certificate.
4. Certificate of Registration in the HP Pharmacy Council alongwith renewal certificate.
5. Certificate of valid SC/ST/OBC on parental basis, if any obtained within the stipulated period from competent authorities, in the prescribed format
6. Sports Achievements.
7. Character Certificate issued by the Magistrate/ Executive Magistrate.
8. Certificate HP Bonafide.
9.Passport size photograph.
